POSITION SUMMARY: With required certification and training, provide certified ADvantage Case Management services though collaborating with ADvantage Members and their support systems, including family members, caregivers, etc., by assessing, facilitating, planning, advocating and monitoring health and social needs, thus enabling members to remain in their home and in the community. Administer certified ADvantage case management services within the scope of practice of the LIFE Senior Services certified program. Work may entail working with high risk clients and helping mentor new certified ADvantage case managers, as appropriate. PRINCIPAL DUTIES AND RESPONSIBILITIES: The following represents the majority of required duties performed by a LIFE Senior Services certified ADvantage Case Manager (ADv/CM), but these are not all inclusive, nor prevent other duties from being assigned as necessary. All job assignments and duties must adhere to Medicaid 1915 C waivers and relevant state requirements under contract audits and instructions.1. Complete comprehensive ADvantage assessments of Member needs and capabilities.2. Collaborate with Members to develop a member-centered plan of care which includes implementation alternatives and effective use of formal and informal community resources.3. Implement, monitor and evaluate Member services to achieve positive outcomes to maintain high quality, cost-effective care, while minimizing fragmented access to services.4. Serve as an advocate for the Member, with an emphasis on encouraging Members to remain as independent for as long as they are able in their own home and in the community.5. Serve as facilitator to promote communication and delivery of quality required and certified services and coordination of these services with all team members, including the member, family, providers and related services, and other agencies through the interdisciplinary team process (IDT's) and other allowed methods.6. Efficiently maintain required documentation and member records in accordance with policies and procedures of contracted programs, meeting all deadlines on time.7. Promote participation of older adults and citizens with disabilities in all aspects of community life through participation in relevant meetings, LIFE community and related activities, etc.8. Participate in required staff meetings, necessary in-service education programs and other continuing education opportunities, and when appropriate and required for certification, licensure, etc.9. Other work as assigned. EDUCATION AND EXPERIENCE: BA, BSN, MA or MS in social sciences, health or any related field with professional experience in case management; current Registered Nurse registration with professional case management work experience. All ADvantage Case Managers must be certified by the State and have one year prior professional experience working with seniors and/or people with disabilities. CERTIFICATION, LICENSES, REGISTRATIONS: Certified Case Manager for ADvantage through OKDHS/ADvantage preferred; Oklahoma drivers' license and auto insurance required. SKILLS: Excellent verbal, interpersonal and written communication skills; Excellent problem solving skills; Ability to use a computer and relevant data bases, especially for required reporting; Ability to organize and manage a caseload in accordance within agency standards and in compliance with policies and procedures of contracted programs; Ability to relate well with seniors and people with diverse disabilities in a variety of settings, e.g., home and community; Knowledge of community resources, disability and aging issues; Ability to meet strict deadlines; Ability to work both independently and interdependently; Ability to work easily as part of the planning team; Ability to manage risk and difficult issues impacting a member; and, when appropriate, Ability to serve as a mentor for less experienced case managers. PHYSICAL REQUIREMENTS: Good hearing and visual capacity. Seventy percent of Case Management work is performed while sitting. Have access to and ability to drive one's own vehicle. WORK ENVIRONMENT: The work environment is in a non-profit, United Way, social services office which provides relevant information, case management and related services to seniors and local citizens with diverse disabilities and chronic conditions and their families and/or caregivers. The work environment also includes some work in the member's home or living arrangement in the community. The work activity level is high and the noise level is moderate.
